ASSESSMENT

An assessment for determining an individual's eligibility for VR, 

services and vocational rehabilitation needs by qualified personnel includes, if  appropriate, an assessment by personnel skilled in rehabilitation technology, and which includes:

  • The identification of employment barriers

  • Determining the nature and extent of the physical or mental impairment's)  and resultant impact on the individual's employability

  • determining the likelihood that an individual will benefit from vocational rehabilitation services in terms of employability

  • Determining an employment goal consistent with the capacities of the individual and employment opportunities  

The assessment services may include but not be limited to:  work sampling, situational assessments, behavioral assessment, independent skills assessment, career exploration, job matching, functional skills, vocational profiles and vocational interest inventories and assessments.

 

 

COUNSELING & GUIDANCE

Counseling and guidance, including personal adjustment counseling, and making those referrals and other services necessary to help and individual secure needed services from other agencies.  Services will also include career planning and case management, development and review of a career plan, job search plan, and/or individual written rehabilitation plan.

 

 

VOCATIONAL AND OTHER TRAINING SERVICES

Vocational and other training services, including personal and vocational adjustment, books, tools and other training materials, except training services in institutions of higher education which are not provided by Employment Resources, Inc.  Training services shall also include job preparation training, job related training, job-site training, and job site advocacy with the employer and/or co-workers.  Additional vocational services may include preparation of a resume, improving interviewing and networking skills and provision of a job club for peer supports.

 

Employment Resources, Inc. will provide guidance and assistance to persons served to understand the impact of employment on disability benefits as well as other benefits and assist him/her to become knowledgeable about access to such benefits.

 

 

SERVICES TO FAMILY MEMBERS

Services to family members of a disabled individual only if necessary to the successful vocational rehabilitation of that individual.  Services may include counseling, meeting, and planning and evaluation of the person's program and/or outcomes.  personal supports may be provided on a time limited basis to assist the family to help prepare the person to go to work.

 

 

WORK-RELATED PLACEMENT SERVICES

Work related placement services to secure suitable employment.  These services include, but are not limited to job placement, job matching, job search assistance, placement assistance, transitional employment services, personal assistance, case management, counseling, medication compliance assistance, job analysis & task analysis.

 

 

POST-EMPLOYMENT SERVICES

Post-employment services necessary to maintain, regain or advance into suitable employment at or above the SGA level.   Post-employment services are critical to the success of the placement of a person at SGA or at any level.  These on-going support services include job site training, educating supervisors and/or co-workers, providing on-call troubleshooting services, self management raining, monitoring and evaluating the employment site to prevent problems, and advocacy on the behalf of the consumer if necessary with the employer.
